How long has CareGuide been in business?
Legacy companies Patient Infosystems, Inc. and Coordinated Care Solutions, Inc. (d/b/a CareGuide) merged on January 25, 2006. Both legacy companies were founded in 1995.

Is CareGuide a publicly traded company?
Yes. Patient Infosystems, Inc. became a publicly traded corporation in 1996. As a result of the merger, CareGuide is now a publicly traded corporation under the symbol PATY.OB.

What kind of services does CareGuide provide?
CareGuide is a national disease and healthcare management company that provides a full range of healthcare management services to a broad cross-section of the healthcare market.

CareGuide brings our clients a highly specialized infrastructure and multi-disciplinary clinical care management staff to reduce the overall costs of care by identifying individuals at risk for hospitalization and other high cost services, developing care management plans centered around self-management, and ensuring that the most appropriate services are utilized. CareGuide’s approach to care management is holistic in nature, recognizing that factors other than physical maladies contribute to an individual’s decline. By providing comprehensive medical and psychosocial care management services for the highest-risk, medically complex members, CareGuide enables customers to realize lower health care costs, while optimizing the quality of care and satisfaction of members.

A fundamental belief of the company is that technology, combined with a personal touch, is an essential component of providing the most effective and efficient care management services. To that end, the company is developing an integrated clinical information technology platform that it believes will lead to the next generation of care management services. Today, the company utilizes technology to predict members at risk for high utilization of medical services, to guide its disease management, care management, and nurse help line interventions, and to communicate with members via remote monitoring devices.

CareGuide strives to individualize and tailor every intervention to the specific needs of the individual. In doing so, our goal is to minimize unnecessary intrusiveness, maximize efficiency through the use of technology and reserve the highest intensity care management services for those individuals who have or are about to have high medical needs and can benefit from our assistance. We recognize the need to use different channels to reach and help different members.

Our suite of care management products are marketed under the label Care Team Connect™ (CTC). Each CTC product is designed to independently yield value for clients, and additional value can be achieved by combining Care Team Connect products for an integrated approach which facilitates timely communication and coordination among patients, providers and payors. The CTC suite includes analytics and predictive modeling, continuous care management, disease management, utilization management, case management, 24/7 nurse help line and maternity management. For higher need members, CTC offers specialized care management that focuses on individuals with the most complex needs who are the highest drivers of healthcare expenditures. Working with network providers and members, this product provides intensive case management for patients who have been discharged from a hospital and are using post-acute services (home care, skilled nursing, etc.). We also have a network of more than 10,000 field care managers who are able to make home assessments and provide other on-site services. Our information-based medicine and state-of-the art, proprietary platform allow us to identify, stratify and assess member needs and determine the interventions and services that will realize the greatest value.

All providers undergo a rigorous screening process that includes extensive applicant profiling to ensure that NCQA and JCAHO credentialing standards can be met.
